Community of Hope is seeking a passionate and dynamic
Perinatal Group Health Educator
to provide group education focused on perinatal health, childbirth, infant care, and parenting to patients participating in group care. This role is an exciting opportunity for someone who thrives in group settings and wants to make a meaningful impact on the health and well-being of individuals and families in our community. This position is located at our Conway Health and Resource Center in Southwest, DC and requires the ability to accommodate a non-traditional work schedule to include regular evenings and weekends. What You’ll Do
Provide group education focused on perinatal health, childbirth, infant care, and parenting to patients participating in group care.
Co-facilitate with providers multiple prenatal and well-child group appointments using the CenteringPregnancy/CenteringParenting model, incorporating the three major components: assessment, education, and support.
Facilitate and assist with the delivery of perinatal and newborn education to support positive health outcomes (e.g., breastfeeding and lactation, childbirth, and newborn care).
Utilize quality management reports to plan and conduct outreach to eligible perinatal patients and pediatric caregivers of patients under 1 year old, using telephonic, in-person, and electronic outreach strategies.
Connect with prenatal patients (by 16-20 weeks) and parents of pediatric patients (by 6 weeks postpartum) to transition from individual medical care to group medical care.
Facilitate monthly group offerings that develop peer connection and support between families at COH (e.g., Parent Café, play groups).
Communicate with providers and guests regarding upcoming group sessions, including notices about changes to groups.
Must-Haves
High School Diploma/GED.
Valid BLS Certificate.
Minimum 2 years of experience in a health-related field.
Experience providing health education in group or individual settings.
Ability to accommodate a non-traditional schedule to include regular evenings and weekends.
Ability to work at multiple sites as groups will take place at three different sites or wherever is convenient for patients.
Nice-to-Haves
Bachelor’s degree in human services, public/community health, health education or social work.
Ability to fluently speak Spanish or Amharic.
Experience coordinating programs or initiatives and/or facilitating groups.
Ability to express enthusiasm about breastfeeding and helping birthing people and families have a positive experience.
Knowledge of the CenteringPregnancy and/or CenteringParenting model (preferred) or experience with perinatal groups.
Commitment and ability to work with underserved populations.
