Senior Home Health Aide

Our senior home health aides are pivotal in our efforts to provide compassionate, top-notch care to our patients in the comfort of their homes. From monitoring a client's vital signs and physical or mental condition to assisting with grooming, hygiene, and chores around the house, our HHAs allow clients to maintain the familiarity - and dignity - of living at home. Responsibilities

  1. Supports a patient's plan of care (POC) by monitoring and recording patient condition, providing support and personal services, and teaching families
  2. Monitors and assesses patient condition by observing physical and mental condition, intake and output, and exercise. Performs patient assessments and collaborates with the multidisciplinary care team to develop and implement a plan of care.
  3. Contribute to the continuity of care through effective communication and partnership with the designated care team.
  4. Supports patients by providing housekeeping or laundry services, shopping or other household tasks, and preparing and serving meals and snacks
  5. Provides personal services as needed by the client, such as bathing, dressing, and grooming
  6. Helps family members care for their loved one by teaching appropriate ways to lift, turn, and re-position the patient
  7. Interface with family members to field concerns and questions, and educate them on the patient's treatment and aftercare needs
  8. Records patient information via our EMR system and notifies branch leadership of any changes or unusual conditions
  9. Enhances service reputation by accepting ownership for accomplishing new and different requests and exploring opportunities to add value to our accomplishments
  10. Performs on-call responsibilities and on-call services to patients/families as assigned with a minimum of 10 hours a week which may include nights, weekend and holidays.
  11. Maintains a safe, secure, and healthy environment by following asepsis and universal precaution controls in performing client care
  12. Protects the agency by adhering to professional standards, home care policies and procedures, and federal, state, and local requirements
  13. Maintain mandatory continuing education requirements (CEUs) for the agency/licensing board.
  14. Must be committed to 30 hours a week as this position is eligible for all company benefits and all financial incentives.

Education & Experience Requirements

  • High school graduate or equivalent
  • Home health aide certificate or other similar certification in good standing from an accredited training course that meets the standards established by Health and Human Services
  • Minimum of one year (1+) of experience as a home health aide or certified nursing assistant (CNA)
  • Strong English language both written and verbal
  • Active CPR certification required
  • Valid driver's license, reliable transportation and insurance required
