Base pay range: $203,400.00/yr - $299,500.00/yr

Our clinics offer a team-based care model where physicians lead a multidisciplinary team supporting physical, emotional, and social wellness of seniors.
Responsibilities
- Evaluate and treat patients according to standards of care.
- Coordinate patient services, including referrals and hospital coordination.
- Participate actively with the care team in daily huddles.
- Ensure timely medical record documentation and adhere to policies.
- Engage in quality improvement and growth opportunities.
- Spend 100% of clinical time on direct patient care.
Qualifications
Required
- Current and unrestricted medical license in the state of practice.
- Graduate of an accredited MD or DO program.
- Excellent communication skills and ability to work in a team-based care model.
- Willingness to participate in TB screening.
Preferred
- Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Active DEA license and Medicare Provider Number.
- Experience with Medicare Advantage and understanding of HEDIS quality indicators.
- 2-5 years of relevant experience.
