Summary
The Hampton VA Medical Center is a 1-C teaching facility serving Eastern Virginia and Northeastern North Carolina. The main campus is in Hampton, Virginia, with four community-based outpatient clinics in Chesapeake, Portsmouth, Virginia Beach, Virginia, and Elizabeth City, North Carolina. Hampton VAMC provides comprehensive health care, including primary care, acute inpatient, acute psychiatric, chronic spinal cord injury, a 169-bed domiciliary, and a long-term Community Living Center.
Qualifications
To qualify for this position, you must meet the basic requirements and any additional requirements listed in the job announcement. Applicants pending completion of training or license requirements may be referred and tentatively selected but cannot be hired until all requirements are met. Current VA-employed physicians who met the initial appointment requirements are deemed to have met the basic requirements.
Basic Requirements:
- U.S. Citizenship (non-citizens only if recruitment of qualified citizens is not possible)
- Degree of doctor of medicine or osteopathic medicine from an approved school
- Current, full, and unrestricted license to practice medicine or surgery in the U.S.
- Completion of residency training approved by the VA, leading to eligibility for board certification
- Residents enrolled in ACGME/AOA accredited residencies are eligible to be appointed as Physician Resident Providers (PRPs), who must be fully licensed and may only be appointed on an intermittent or fee basis
- Proficiency in spoken and written English
- Preferred experience includes geriatric clinical practice, primary care, and home care/home visits
- Board certification in Geriatric Medicine and/or Palliative Medicine is highly desirable
- Physical ability to perform all activities of the position without restrictions
Duties
- This is an open continuous announcement; all qualified applicants will be referred weekly
- The Home-Based Primary Care physician manages all aspects of geriatric primary care within the section and supervises staff
- Responsible for planning, organizing, developing, implementing, and refining comprehensive patient management and clinical support within HBPC
- Provides services, technical counsel, and leadership guidance to meet patient care needs
- Embraces Veteran-centered geriatrics, palliative, and hospice care
- Maintains records of meetings, reports, data, and evaluations
- Develops policies and procedures to ensure effective management, ethics, safety, communication, and quality
- Reviews and recommends modifications to privilege templates annually
- Provides clinical judgment and administrative capabilities for diverse patient populations and interdisciplinary teams
- Coordinates Veteran care with VA and non-VA entities
- Performs evaluations, diagnosis, and treatment of patients within HBPC
- Participates in team meetings and treatment planning
- Orders diagnostic tests, discriminates findings, and makes treatment decisions
- Completes clinical documentation timely and accurately
- Ensures practice compliance with accreditation, VA policies, clinical guidelines, and regulations
- Performs other tasks as assigned by the Chief of GEC and the Medical System
- Responsible to the Chief of GEC or supervisor
