The Primary Care Physician (PCP) at CenterWell works as a leader within our team-based care environment. We are a value-based care provider committed to quality care for our patients. Our care team includes Doctors, Advanced Practice Professionals, Pharm D, Care Coach Nurses, Medical Assistants, Behavioral Health Specialists, and more. Responsibilities:

  1. Evaluate and treat patients in accordance with standards of care.
  2. Monitor care quality using data and chart reviews.
  3. Coordinate patient services, including referrals and hospital/SNF coordination.
  4. Participate actively in daily team huddles as a medical expert.
  5. Support a cooperative practice environment through professional demeanor.
  6. Complete medical records timely, working with coders to optimize coding specificity.
  7. Follow policies and protocols set by Clinical Leadership.
  8. Meet regularly with the Regional Medical Director regarding care quality and records.
  9. Engage in growth opportunities for new or existing services.
  10. Participate in the local primary care on-call program as needed.
  11. Ensure compliance with licensing, certification, and accreditation standards.
  12. Spend 100% of your time on direct patient care, including administrative tasks related to patient care.

Required Qualifications:

  • Current and unrestricted medical license or willingness to obtain one in the practice state(s); eligible to obtain licenses in other states as required.
  • Graduate of an accredited MD or DO program.
  • Excellent communication skills.
  • Strong interpersonal skills with colleagues and patients.
  • Commitment to an integrated, team-based care model.
  • Willingness to learn and adapt to a value-based care setting.
  • Superior patient/customer service skills.
  • Basic computer skills, including email and EMR.
  • This role is patient-facing and part of our TB screening program; screening for TB is required if selected.

Preferred Qualifications:

  • Board certification or eligibility (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Active and unrestricted DEA license.
  • Medicare and Medicaid Provider Numbers.
  • 2-5 years of relevant experience preferred.
  • Experience managing Medicare Advantage panels and understanding of coordinated care in a value-based environment.
  • Knowledge of Medicare guidelines and coverage.
  • Bilingual skills are a plus.
  • Knowledge of HEDIS quality indicators.

Scheduled Weekly Hours: 40

Pay Range: $203,400 - $299,500 annually, based on location and experience. Eligible for bonus incentives.
