About the role

  • Seeking a clinically accomplished, mission-driven physician leader as Market Medical Director for Baltimore. This role involves community engagement and operational leadership to develop practices across Baltimore centers.

  • Smaller Panels, Deeper Care: PCPs manage no more than 400 patients for continuity and proactive care.

What you'll do

  • Lead and coach care teams to deliver excellent, compassionate, and cost-effective care.
  • Foster provider engagement and professional growth.
  • Ensure efficient workflows, timely follow-up, and equitable scheduling.
  • Engage with patients, families, and community partners to build trust.
  • Maintain a small patient panel for frontline care and best practices.
  • Monitor clinical performance, quality, utilization, and patient experience.
  • Support recruitment, onboarding, and retention of clinical staff.
  • Represent Sage in community and regional meetings.
  • Ensure compliance and contribute to policy improvements.

Qualifications

Required

  • MD or DO with active or eligible Maryland licensure.
  • Board certification in Internal Medicine, Family Medicine, or Geriatric Medicine.
  • At least 8 years of clinical experience, including work with complex seniors.
  • Minimum 5 years of clinical leadership experience, preferably in value-based or risk models.
  • Experience with underserved and diverse communities.
  • Knowledge of CMS quality programs, risk adjustment, and Medicare Advantage.
  • Strong communication and team-building skills.

Preferred

  • Experience in Medicare Advantage or risk models.
  • Knowledge of HEDIS, STARS, CAHPS, HOS, and RAF scoring.
  • Success in improving documentation and provider performance.
  • Experience integrating behavioral health and social determinants into primary care.
