Ash & Harris Executive Search is looking for a Hospice RN Case Manager
Overview:
The Hospice RN Case Manager is a registered nurse responsible for providing compassionate, patient-centered care to individuals with life-limiting illnesses. This role involves coordinating and delivering comprehensive nursing care, managing symptoms, and supporting patients and their families through the end-of-life process. The RN Case Manager serves as the primary point of contact for the patient’s care plan, collaborating with an interdisciplinary team to ensure physical, emotional, and spiritual needs are met.
Key Responsibilities:
Conduct comprehensive assessments of patients’ physical, emotional, and psychosocial needs.
Develop, implement, and update individualized care plans in collaboration with the hospice team.
Administer medications and treatments to manage symptoms (pain, nausea, anxiety, etc.).
Educate patients and families on symptom management and end-of-life care.
Act as the primary liaison between patients, families, and the interdisciplinary team.
Provide emotional support and counseling on advance care planning and grief.
Maintain accurate documentation in compliance with regulatory standards.
Attend interdisciplinary team meetings to review and adjust care plans.
Requirements:
Education:
Associate or Bachelor’s degree in Nursing (BSN preferred).
Current, unrestricted RN license in the state of practice.
Certification in Hospice and Palliative Nursing (CHPN) preferred (or willingness to obtain).
Experience:
1–2 years of nursing experience (hospice, palliative care, oncology, or geriatrics preferred).
Case management or care coordination experience is a plus.
Other:
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal abilities.
Ability to work independently and manage a caseload efficiently.
Proficiency in electronic medical records (EMR) systems.

$100,000 – $103,000 Per Year Benefits:
