Travel Requirements:
This role involves regular travel in Monroe, Washtenaw, Western Wayne, and parts of Oakland & Livingston Counties providing Home Based Care.
Huron Valley PACE® serves Washtenaw, Monroe, Western Wayne, and parts of Oakland and Livingston counties providing participants a wide range of medical and supportive services in our Day Clinic located in Ypsilanti, Michigan or in the comfort of their own homes. The Huron Valley PACE team of healthcare professionals specialize in working with the senior community to help participants and their family make positive healthcare decisions and receive the care they deserve.
What You'll Do in This Role:
As a
Participant Care Assistant , you'll play a hands-on role in making a real difference every day. You'll support participants with their care needs- in their homes, while working closely with an incredible team of healthcare professionals. Under the guidance of the Participant Care Manager, you'll use your skills and compassion to provide meaningful support that promotes comfort, dignity, and independence. Primary Responsibilities:
Support Comfort & Confidence : Help create a positive, caring environment by supporting participants' physical and emotional well-being.
Encourage Independence : Empower participants to stay as independent as possible by supporting their daily routines and mobility goals.
Be a Communication Pro : Keep the conversation flowing-whether it's with participants, their families, your team, or partner agencies.
Collaborate with the Care Team : Be an essential part of our interdisciplinary team, sharing insights and ideas to improve care.
Provide Personal Care with Purpose : Assist with essential daily tasks like dressing, toileting, transferring, and mobility-helping participants feel their best.
Keep it Clear & Accurate : Document the care you provide, changes you observe, and any updates in participant activities with precision.
Speak Up for Participants : Notice a change in a participant's condition? You'll be the voice that ensures they get the care they need.
Grow with Us : Play an active role in quality improvement initiatives that enhance services and outcomes.
Learn & Teach Along the Way : Whether you're gaining new skills or sharing your own knowledge, you'll be part of a team that supports continuous learning.

What You'll Need:
A high school diploma or GED-your foundation for success • Must be at least 18 years of age. At least 1 year of experience supporting older adults or frail individuals-you know how to care with compassion Completion of Huron Valley PACE's Participant Care Aide (PCA) program-your training sets you apart
