YoloCares CalAIM And Supportive Care Outreach Coordinator
YoloCares CalAIM and Supportive Care Outreach Coordinator will play a crucial role in supporting the implementation of the CalAIM and Supportive Care initiatives by coordinating outreach efforts to engage eligible individuals and communities. This role involves extensive community engagement, partnership building, and collaboration with various stakeholders to promote awareness and access to CalAIM and ECM services. Essential Duties And Responsibilities Advocates for and stays abreast of current and evolving needs of underserved and historically marginalized patient populations. Assists individuals in understanding CalAIM and ECM services and eligibility criteria. Coordinates referrals for community services, client needs assessments and coordinates initial admission plan of action. Understands referral processes, health forms, diagnosis, and CalAIM processes clearly enough to articulate to the patient, family or case manager with cultural responsiveness. Documents all assessments and interactions with patients in a timely manner. Develops and implements outreach strategies to identify and engage eligible individuals for ECM services. Works directly with ODEIB to conduct community presentations, workshops, and informational sessions to raise awareness about ECM and YoloCares services. Establishes and maintains relationships with community-based organizations, healthcare providers, local government agencies, and other stakeholders. Engages and maintains strategic partner pipeline and collaborates with established partners to coordinate outreach activities and leverage resources. Provides face-to-face support to individuals and families to navigate the enrollment process for CalAIM services. Monitors and evaluates the effectiveness of outreach strategies and makes recommendations for improvements. Participates in training and professional development opportunities related to CalAIM, ECM, priority population, and outreach best practices. Prepares and presents regular reports on outreach efforts and outcomes for internal and external stakeholders. Informs the Administration of any illegal or harmful (or potentially so) situations regarding the operation of programs and clients and utilizes appropriate judgement when engaging with patient, family or event. The schedule for this position is Monday through Friday from 8:30 am to 5:00 pm with some weekends required. Requirements Has lived experience personal or professional with the homeless population. Has or is willing to obtain Community Healthcare Worker Certification within 18 months of employment. High school diploma or equivalent or additional education or background in Public Health, Social Work, Community Development, or a related field is preferred. Experience in community outreach, public health, social services, or other related fields, preferably with a focus on engaging diverse populations. Sufficient understanding of the CalAIM and Palliative Care initiative, its goals, and the services offered. Familiarity with social determinants of health and community resources available to target populations. Strong communication and interpersonal skills for effective community engagement and partnership building. Lived experience navigating housing insecurity, caregiving, chronic health issues, or barriers to care. Deep understanding of how poverty, race, language, or immigration status impact people's access to services. Awareness of and sensitivity to the cultural, linguistic, and socioeconomic diversity of the communities served. Comfortable speaking with people from all walks of life, including seniors, caregivers, immigrants, and unsheltered individuals. Ability to develop outreach strategies and materials that resonate with diverse audiences and accurately reflects the organization's objectives and services offered. Proficiency in research processes, data collection, and analysis to assess outreach effectiveness and inform strategies. Ability to make informed decisions regarding outreach methods and engagement tactics based on community needs and reception. Credentials or relevant training in community engagement strategies, cultural competency, specific services offered under Palliative Care, CalAIM, and/or DHCS best practices. Able to work in partnership with both outreach and clinical teams. Bilingual or multilingual is a plus! Physical And Other Job Specifications Able to hear, speak, read, write, and/or type well enough to perform required job duties. Able to use hands to finger, handle, or feel. Able to occasionally walk and reach with hands and arms. Able to lift and carry up to 35 pounds. Specific vision abilities required by this job include close vision and ability to adjust focus.
