Nurse Navigator, Medicine (Gastroenterology)

Under general administrative direction, the Gastroenterology (GI) Nurse Navigator coordinates and manages treatment plans for patients with gastrointestinal conditions. This role provides a significant level of patient education related to illness management and treatment, facilitates communication among multidisciplinary teams, ensures continuity of care, and optimizes patient outcomes. The GI Nurse Navigator supports patient care coordination, assists with clinical follow-up, orders necessary labs and tests, and addresses barriers to effective care delivery. Essential Duties and Responsibilities: Serve as a liaison between patients, GI providers, and healthcare teams to ensure seamless coordination of care. Address barriers to care, including scheduling challenges, insurance concerns, and patient education needs. Educate patients on gastrointestinal conditions, treatment plans, medication adherence, dietary considerations, and follow-up care. Follow up on diagnostic tests and communicate results and next steps to patients. Provide clinical support, counseling, and education; escalate urgent clinical situations to providers as needed. Collaborate with the GI team to develop and implement care plans focused on improving patient outcomes. Ensure thorough documentation of patient encounters, laboratory results, and treatment plans in the electronic medical record (EMR). Work with the multidisciplinary team to reduce readmissions and address chronic disease management and social or environmental factors impacting patient care. Assist GI providers in addressing any gaps in care by coordinating follow-ups, procedures, and referrals. 5% of responsibilities will involve direct clinical duties, including ordering labs, reviewing patient records, and supporting procedures as needed. Minimum Qualifications: Education: Bachelor's degree in Nursing (BSN) required. Current D.C. licensure as a Registered Nurse (or evidence of application for licensure in D.C.). Experience: Minimum of 2 years of clinical nursing experience, preferably in gastroenterology, internal medicine, or patient education settings. Physical Requirements: Ability to walk, stand, and reach above shoulder level frequently in a clinical setting. Must be able to occasionally lift or carry, and frequently push or pull up to 100 lbs. Regular exposure to healthcare environments requiring personal protective equipment. Manual dexterity to operate computers, calculators, copiers, and other standard office equipment.
