The Social Services Specialist responsibilities include: Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community Provides coordination, referral and interdisciplinary discharge planning They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs Possesses a clear understanding of discharge and transition planning and linkage with community resources Demonstrates skills in planning, organizing, and managing multiple functions Position details: Location: Dunedin Hospital - Dunedin, FL Status: Part Time, 16 hours per week Schedule: Saturday & Sunday 10:00 AM - 6:30 PM Weekend Requirement: Every On Call: No Certifications and Licensures: Preferred ACM (Case Management) or Preferred CCM (Case Manager) Education: Required Bachelors in Social Work, Sociology, or Related Field Experience: Required 6 months Internship or Required 6 months Social Work in an acute care setting Benefits: Benefits (Health, Dental, Vision) Paid time off Tuition reimbursement 401k match and additional yearly contribution Yearly performance appraisals and team award bonus Community discounts and more Equal Opportunity Employer Veterans/Disabled
