Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs. Qualifications

Bachelor's degree or equivalent required. Master's degree or equivalent preferred. 2 years of relevant experience required.
