Field Chaplain - Hospice

The Field Chaplain will be responsible for the spiritual care assessment and participate in the development of the interdisciplinary plan of care for the terminally ill hospice patients and their families, as assigned. The Chaplain will ensure the best spiritual outcomes as the patient desires or requests. Responsibilities include: Providing and documenting spiritual care assessments Providing spiritual support and services to the patient/caregiver Facilitating contact with community spiritual support Assessing bereavement needs Providing crisis intervention Serving as a resource to the interdisciplinary team Participating in education of the community regarding Hospice care Providing direction and supervision to assigned Spiritual Care Volunteers Required qualifications: Masters of Divinity, M.Div., required Member of the College of Chaplains One basic unit of completed CPE required Sanctioned by a recognized religious organization Previous experience in Hospice or a health care setting preferred Excellent time management, organizational skills, self-awareness, and boundaries which are modeled for others Valid driver's license with a good driving record; personal car in good working order
