RN Transitional Care Navigator, Population Health

Hourly Pay Range: $40.45 - $62.70 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights: Position: RN Transitional Care Navigator, Population Health Location: Elmhurst, IL Full Time/Part Time: Full Time, 40 hours per week, day shift Hours: Monday-Friday, 8:30a-5:00p, rotating weekends may be required in the future Required Travel: Expected to travel to meetings at assigned skilled nursing facilities and hospitals as needed. What You Will Do: The RN Transitional Care Navigator

Population Health is responsible for supporting high-risk patients who have been discharged to a skilled nursing facility. This role focuses on managing the length of stay in the sub-acute setting and minimizing readmissions for patients requiring post-acute care.
