The IBH Care Coordinator plays an essential role on the Integrated Behavioral Health (IBH) team, supporting care coordination efforts across primary care, behavioral health, housing, and social service systems. This position is ideal for a Licensed Professional Clinical Counselor Associate seeking hands-on experience in an integrated care environment focused on improving health access and outcomes for low-income communities in Alameda County. Supervision toward clinical licensure is available for qualified candidates. Key Responsibilities
Care Coordination & Program Support Deliver in-person and telehealth-based care coordination for individuals with behavioral health and social support needs Conduct comprehensive client assessments and contribute to individualized care planning Facilitate warm handoffs and referrals to behavioral health, medical, housing, and community-based resources Monitor engagement and support follow-up care to maintain service continuity Collaborate with internal care teams and external partners to address client needs holistically Participate in multidisciplinary team meetings, case reviews, and service planning sessions Document services and progress accurately in the electronic health record (EHR) Assist with data collection and service tracking to meet reporting requirements Compliance & Training Ensure compliance with Medi-Cal, HIPAA, and internal documentation protocols Attend required trainings, community forums, and IBH-related meetings Support program evaluation activities by monitoring outcomes and service delivery Contribute to outreach, resource navigation, and sustainability efforts
