Job Description Conduct an admission interview with the resident and family for the purpose of completing a confidential psychosocial history and assessment, identify areas of concern, develop treatment plans/goals, and complete advance directives. Update residents' charts/social services records every three months following initial evaluation and/or as social services needs are demonstrated. Re-evaluate resident care plans at least every three months. Maintain periodic contact with families to keep them informed of residents' needs. Assist residents with clothing purchases, obtaining telephone and other items as needed. Facilitate repair of prostheses and arrange for transportation. Advocate residents' rights according to Title XXII. Consult with appropriate staff regarding resident needs and concerns providing resources. Assist with discharge planning in consultation with Supervisor and Director of Nursing. Contribute to Resident Care Conferences, and communicate resident and family areas of concern. Prepare confidential cases or concerns for monthly conference meetings with the Social Work Consultant. Review Advance Directive (AD) and/or Preferred Intensity of Care (PIC) with new residents and their families and follow-up with completion of AD and/or PIC. Assist nursing with dental, vision, hearing, and podiatry needs of the residents.
