Description

POSITION SUMMARY:

The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of registered nurses and healthcare providers, focusing on the physical and mental well-being of patients. LPNs administer medications, monitor patient behavior, and assist with various treatments and therapies. Knowledgeable of a wide range of behavioral health disorders. Participates in the department's performance improvement and continuous quality improvement initiatives.

RESPONSIBILITIES Observe and document patient behavior, including vital signs, response to medication and treatment. Administer prescribed medications, including injections, and appropriately documenting administration in electronic medical record and automated medication dispensing machine. Provide education to patient, including families, about medications, treatment plans and overall health. Collaborate with all members of the treatment team to develop and implement individualized care plans. Respond to emergency situations and provide intervention, including basic life support/first aid as needed. Communicates with patients/families, and co-workers to promote effective relationships. Reports change in patient's condition immediately and communicates other pertinent patient changes throughout the shift to the healthcare provider and registered nurse. Clearly communicates and performs responsibilities in a cooperative, respectful, positive and helpful manner. Assist patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner. Refrains from personal conversations or cell phone use in the presence of patients/family members. Follow proper reporting procedures for actual or potential accidents and/or incidents so follow-up and/or prevention can occur. Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated. Provides for the safety and security needs of patients and staff. Demonstrate use of de-escalation techniques when caring for agitated or violent patients. Maintains current de-escalation training Promptly responds to unsafe incidences to secure patient and protect their wellness. Possesses/demonstrates knowledge of emergency response actions by cooperatively responding and assisting quickly to call bells or any other emergency alarm. Always uses proper body mechanics. Consistently uses the appropriate Personal Protective Equipment (PPE). Record / report the need for service maintenance or repair of equipment and remove any faulty equipment from service.

Qualifications

WORKING CONDITIONS: Well-lighted, well-ventilated work areas. Physically engaged to include, but not limited to the following: Sits, stands, walks, bends and stoops throughout the workday; required to lift to 20 pounds independently using safe lifting practices. Possible exposure to bodily fluids and infections. Deals with residents, families and staff under often stressful conditions.

PERSONNEL SPECIFICATIONS: Graduate of approved school of licensed practical nursing Current license to practice in the State of Connecticut A minimum of one (1) year experience, preferably in a behavioral health setting Current Basic Life Support certification Ability to communicate in English, both verbal and written Basic computer knowledge Strong interpersonal skills
