Practice Setting: Facilities: Post-Acute Care, Long-Term Care, Independent and assisted living Average Number of Patients Per Day: 5-7 for 2- or 3-day request Number of Physicians in Practice: 1 clinic physician Number of Mid-Levels that Provide Coverage: 2 Number of Exam Rooms: N/A - patients seen in their listed rooms: sub-acute, assisted living, and independent Services Available On-Site: Facility Resources EMR: Epic, point click care, quickmar Trauma Level/Designation: sub-acute, assisted living, and independent Other Geriatricians Provide Coverage During Locums Coverage Dates: The first week of 7/28, there may be a Geriatrician who is able to help round prior to maternity leave. Areas of Treatment Required: Age-Related Macular Degeneration (AMD) Alzheimer's Disease Cataracts Glaucoma Major Depression Osteoarthritis Psychosocial Factors Stroke (Brain Attack) Dementia Incontinence Hearing and Vision Loss Cancer Insomnia Diabetes Heart Failure Frailty Balance Issues Osteoporosis Dates of Coverage: 7/28/2025-8/15/2025 8a - 5p M, W, F Locum will be required to float between the two Muskegon sites below. (No floating during the shift)
