Founding team is in NYC. Their preference is onsite in Long Island, NY. However, they're open to remote candidates with the right experience.

Our client is a PE-backed platform preparing to build a nationwide Home & Community-Based Services (HCBS) organization. They are seeking a Chief Growth Officer who is ambitious and ready to scale the company across all 50 states through organic de-novo launches and rapid M&A. If you have experience taking HCBS or home-health entities from application to first claim in multiple states, this is your green-field opportunity. The ideal candidate has 10+ years of experience building or flipping HCBS / HHA entities in at least two states.

Location

  • Preferred: On-site with the founding team on Long Island, NY (easy commute from NYC).
  • Open to Remote: Exceptional applicants with a proven multi-state HCBS / HHA track record will be considered anywhere in the U.S. (25–30% travel).

Core Responsibilities

  1. Operational Launch & Scale
  2. Own every state filing, readiness checklist, survey, and first-claim timeline.
  3. Implement EVV, scheduling, payroll, and claims systems across de-novo and acquired agencies.
  4. Regulatory & Clinical Compliance
  5. Serve as Chief Growth Officer for all state, CMS, and managed-care requirements.
  6. Lead QAPI, incident management, and survey preparedness efforts.
  7. M&A Execution
  8. Partner with PE sponsors on diligence; translate LOIs into operational plans for day one.
  9. Integrate people, processes, and technology while maintaining clinical quality.
  10. Payer & Referral Contracting
  11. Negotiate and network with Medicaid MCOs, Medicare Advantage, PACE, VA Community Care, AAAs, and discharge planners.
  12. People & Culture
  13. Recruit and coach regional leaders; foster a compliance-first, data-driven, yet human-centric culture.

Must-Have Qualifications

  1. 10+ years building or turning around HCBS / Home-Health agencies in two or more states.
  2. Experience taking at least two entities from initial application to first paid claim (licensure, waiver enrollment, MCO credentialing, EVV go-live).
  3. Proven success scaling through de-novo launches and M&A integration.
  4. Deep knowledge of §1915(c) / (k) waivers, CMS-855A, accreditation standards, HIPAA, EVV, incident reporting, and QAPI.
  5. Track record negotiating multi-state MCO / MA contracts and aligning them with operational economics.
  6. Leadership style that balances urgency with process discipline—able to innovate quickly and pass surveys.
