Join Our Compassionate Team as a Licensed Master Social Worker (LMSW)
We are seeking a dedicated and licensed social worker to become an integral part of our interdisciplinary team. As an LMSW, you will provide essential psychosocial support to patients and their families, ensuring their needs and goals are met through comprehensive assessments and established care plans.
Key Responsibilities:
Assess the psychosocial needs of patients and families.
Provide interventions and services based on patient care plans.
Leverage professional training and personal judgment to monitor ongoing psychosocial assessments.
About You:
Master's Degree in Social Work from a CSWE accredited institution.
State licensure as required where our hospice operates.
A minimum of one year of experience as a social worker in healthcare or hospice settings is mandatory; three to five years is preferred.
Deep understanding of the needs of terminally ill patients and their families, including the ability to provide support during crisis situations.
