Position Description:
The Intake Coordinator works closely with the Client Coordinator and other staff members to guarantee accuracy and timeliness of patient care. The Intake Coordinator is responsible for gathering and recording the initial intake of patient needs and assisting with case coordination to ensure a high degree of customer service. The Intake Coordinator is also responsible for daily scheduling and staffing of client services as well as assisting with completion of evaluation of services including customer service calls, observation visits, and managing patient and family requests for change.
Essential Duties and Responsibilities:
Captures patient intake referral information and communicates information to appropriate management teams
Processes and documents incoming patient needs, requests and changes for services
Accurately informs patients of services available
Collaborates with operations team to schedule appropriate personnel in a timely manner without lapses or delays
Assists with new case openings in patient homes
Records and reports patient and employee concerns, complaints and changes in condition to management teams including appropriate payer and case manager
Enters all appropriate patient information into the system of record per company policies and procedures
Assists office staff members with routine office duties
Performs other duties as assigned
Minimum Requirements:
Flexibility to travel; up to 25% travel required
High School Degree (or equivalent) and current Licensed Practical Nurse (LPN) licensure preferred; one (1) year of experience and/or College Degree preferred
Must be a self-starter with the ability to meet deadlines and excel in a fast-paced team environment
Must be organized with strong attention to detail
Strong time management skills
Leadership and customer service capabilities required
Public relations ability, interpersonal skills, and professional telephone manner required
Excellent communication skills
Proficiency in the English language required
Computer proficiency, including Microsoft Office Suite (Word, Excel, Teams, etc.)
Compliance & Ethics Expectations:
Participates and successfully completes the company's compliance program requirements and adheres to the Code of Conduct, Company policies, and applicable federal and state requirements
Sets an example for other employees regarding how the Company's Code of Conduct and Compliance Program is applied and observed every day when dealing with customers, business operations, or other team members
Reports potential violations of company policy, Code of Conduct, and/or applicable laws and regulations to the company hotline, through the chain of command, to the Compliance and Ethics Department, or through other channels made available by the company for reporting potential violations
Promotes an environment in which other employees are encouraged to report potential violations
As appropriate, provides input and suggestions regarding areas in which policies, procedures, workflows, and/or controls can be improved to enhance compliance
Physical/Work Environment Requirements:
Prolonged periods of sitting at a desk and working on a computer
Must be able to lift 15 pounds at times
Compensation:
$24 - $25 per hour, Plus $2,000 annual bonus potential.
