The
Bridge and Engagement Services Care Management Team
(BEST CM) is a post crisis team that works with individuals with acute needs that are not connected to care. The BEST CM team is a field-based team that provides assertive outreach to individuals leaving the emergency room, crisis care or jail. We provide short term behavioral health support and system navigation to support individuals with linkage to the appropriate level of care and to resources to support wellness and stability in the community. MINIMUM QUALIFICATIONS Possession of a valid license issued by the California Board of Behavioral Sciences (BBS) for one of the licenses listed: Licensed Clinical Social Worker (LCSW) Associate Clinical Social Worker (ASW) Licensed Marriage and Family Therapist (LMFT) Associate Marriage and Family Therapist (AMFT) Licensed Professional Clinical Counselor (LPCC) Associate Professional Clinical Counselor (APCC) Complete with application:
