Hematology/Oncology Physician

Coverage Needed:

• Schedule: Monday - Friday, 8am-4:30pm
• Specific Dates:
  • Sept: 5, 12, 15, 16, 19, 22, 23, 26
  • Sept 29-Oct 3
  • Oct: 10, 13, 14, 17, 20, 21, 24, 31
  • Nov: 7, 10, 11, 14, 21, 24, 25
  • Dec: 1, 2, 5, 8, 9,12,19, 26

Clinical Information:

• Outpatient Setting
• No Call or Rounding
• EMR: Epic
• Patient Volume: 20-25 patients per day
• Support : 5 RNs, 2 MA's, 1 Patient Service Representative
• Emergency response skills from side effects of medication
• Chemotherapy treatment plans

Required:

• Board Certified in Hematology/Oncology
• Active IN License and DEA
• CV must be in Month / Year format
• Must have license in hand at time of submission, or proof that the license application has been submitted
• Any gaps greater than 30 days will require an explanation on the CV
• DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
