Implement therapeutic programs to improve mobility, relieve pain, and prevent or limit permanent physical disabilities. Collaborate with nurses, physicians, and occupational therapists to optimize patient outcomes. Document patient progress and adjust plans as needed. Educate patients and families on recovery strategies and exercises.Qualifications: Active Physical Therapist (PT) license in the state of Washington Graduate of an accredited Physical Therapy program BLS Certification (preferred) Prior SNF or geriatric experience is a plus Ability to work independently and as part of an interdisciplinary team Strong communication and organizational skillsWhy Join Connected Health Care?
