YOUR ROLE As a Palliative Care Nurse Navigator at Thyme Care, you will be a vital clinical resource for members with serious illness and their care network. Your responsibilities include conducting conversations about goals and values, advance care planning, and palliative care via phone, email, and video. Under the guidance of our Nurse Team Lead, you will perform symptom assessments, support longitudinal care, and facilitate end-of-life discussions when appropriate. You will actively engage with members, address clinical issues, and manage escalations. The shifts for this position are either 8:30 - 5 EST or 11:30 - 8 EST, with occasional Saturday coverage expected. Within your first three months, you will: Familiarize yourself with Thyme Care systems, tools, technology, and partners, conducting at least 20 member calls per day. Collaborate with Nurse leaders and Medical Directors to ensure alignment with clinical protocols and best practices. Establish trusting relationships with members and their care networks, emphasizing empathy and active listening. Adhere to policies, procedures, and documentation standards. Support members with serious illness and at end-of-life, coordinating care and providing clinical support. Participate in case conferences to monitor progress and collaborate on support plans. Partner with payers and providers to optimize care delivery and reduce readmissions. Work with non-clinical team members to address social determinants of health, such as food and transportation resources. Be available for urgent escalations and provide clinical consults as needed. Bachelor of Science in Nursing, an unrestricted RN license (preferably in NY, IL, MI), with at least 5 years of nursing experience, including 3 years in outpatient palliative care or home hospice for oncology patients. Certifications:

Certification such as CHPN, OCN, AOCN, AOCNS, or CCM is a plus but not required. This description provides an overview of the role, responsibilities, and qualifications.
