We are looking for a Nurse Evaluator to join us full-time in Suffolk County! The Personal Care Attendant (PCA) Program is a MassHealth program that allows people with long-term disabilities to live at home independently. The Northeast Arc Personal Care Management (PCM) program serves about 5,000 individuals living in communities throughout northeastern Massachusetts, Greater Boston, and the Metro West area. Our Skills Trainers, with the help of a nurse and occupational therapist, assess the personal care needs of the individual and prepare a PCA application to submit to the consumer’s insurer. The consumer’s insurance (MassHealth/Medicaid, SCO or One Care Program) grants final approval of the recommended Care Plan. Once approval is received, the Skills Trainer teaches the individual how to successfully hire, train, supervise, and pay his or her own Personal Care Attendants. Job Summary:

The Nurse Evaluator completes essential clinical re-evaluations of PCM consumers to determine program eligibility and appropriate hours of care required in the Cape Anne Service Area. Service area includes the following towns and cities: Arlington, Belmont, Boston, Brookline, Cambridge, Chelsea, Everett, Revere, Somerville, and Winthrop. Major Responsibilities: Conducting face-to-face assessments with potential members in the individual's home or place of residence alongside an Occupational Therapist Evaluating members' physical and cognitive condition and resulting functional limitations to determine the members ability to benefit from and manage PCA services Completing evaluations that accurately represent the member's need for physical assistance with ADLs and IADLs Communication and collaboration with OTs , Skills Trainers, and other team members all pertinent to communication with the PCM Qualifications: Must be fully licensed and must maintain licensure from the Massachusetts Board of Registration in Nursing or LPN Minimum of 2 years nursing experience Experience working in the direct care of elderly or disabled adults Home-care experience a plus Completion of Mass Health PCM Training program CPR certification Understanding of community supports Valid driver's license and vehicle Job Type: Per diem, flexible schedule Pay Rate: $85/per evaluation, with training hourly rate.
