JOB SUMMARY:We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our skilled nursing facility. The LPN will be responsible for delivering quality patient care in compliance with facility policies, state regulations, and healthcare standards. This role is crucial in ensuring the health, comfort, and safety of our residents.RESPONSIBILITIES:Assists with new admissionsProvide medical intervention as needed for emergent needsCommunicate needs of residents to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etcProvide direct nursing care to residents, including administering medications, treatments, and monitoring vital signsPerform wound care, dressing changes, and other therapeutic nursing interventions as directedObserve residents for changes in health status and report any concernsAssist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and feedingCollaborate with interdisciplinary teams to ensure quality resident careMaintain accurate and timely documentation of resident care, treatments, and progress in electronic medical records (EMRs) or paper chartsEnsure compliance with all federal, state, and local regulations as well as facility policies and proceduresProvide emotional support to residents and their families, fostering a compassionate and empathetic environmentParticipate in staff meetings, in-service training, and continuing education programs to stay updated on best practicesSupport infection control and safety protocols throughout the facilitySupervise certified nursing assistants (CNAs) and provide direction as neededOther duties as assignedREQUIREMENTS:Current Licensed Practical Nurse (LPN) license in the state of practiceExperience in a skilled nursing facility or similar healthcare setting (preferred)Basic Life Support (BLS) certification requiredStrong understanding of geriatric care and the needs of elderly residentsProficiency in using electronic health records (EHR) systemsExcellent communication and interpersonal skillsAbility to work in a fast-paced environment and handle multiple tasks with attention to detailCompassionate, patient-focused, and empathetic approach to careEMPLOYEE BENEFITS:Benefits of being a Sapphire Health Services employee include, but is not limited to:PTO401(K)Medical/Health InsuranceDental Insurance Vision InsuranceBirthday/Holiday PayWellness FundUniform FundLongevity BonusesQuarterly Vacation DrawingTuition ReimbursementHOW TO APPLY:At Sapphire Health Services we are dedicated to creating a supportive and enriching environment for both our residents and our team members.
