Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program approval, including award amount (up to $200,000) and eligibility period (one to five years), is determined by the VHA Education Loan Repayment Services program office after review of the application. Former EDRP participants are ineligible to apply. Qualifications

Basic Requirements:

United States Citizenship: Non-citizens may only be appointed when qualified citizens cannot be recruited, in accordance with VA Policy. Education: Have a master's degree in social work from a school accredited by the Council on Social Work Education (CSWE). Graduates from schools in candidacy status do not meet this requirement until fully accredited. A doctoral degree in social work cannot substitute for the master's degree. For the GS-12 level, candidates need: One year of experience at the GS-11 level demonstrating advanced practice skills and judgment. Possession of certification or post-master's training from a recognized organization or university, including a defined curriculum and internship, or equivalent supervised experience. Licensure at the advanced practice level and ability to supervise licensure. Knowledge, Skills, and Abilities (KSAs): Knowledge of program coordination, administration, consultation, negotiation, and monitoring. Ability to write policies, procedures, and practice guidelines. Supervisory skills for multidisciplinary staff. Organizational skills to manage priorities and meet deadlines. Training and guidance skills within clinical practice. Additional Requirements

Physical demands include walking, sitting, talking, driving, and telephone use. Light lifting and carrying under 15 pounds; hearing aid permitted. Travel up to 50% to various settings including medical centers, community-based adult day health care sites, skilled nursing facilities, and ombudsman offices. Must possess an active California driver's license and maintain it in good standing. Duties

Oversee clinical and programmatic aspects of Veterans in VA-contracted skilled nursing and adult day health care settings. Ensure compliance with VHA directives and handbooks. Conduct monthly oversight visits, coordinate inspections, address issues raised by Veterans or families, advocate for quality care, participate in quality improvement meetings, and maintain partnerships with local ombudsman offices. Serve as a liaison among Veterans, their families, community facilities, and the SF Veterans Health Care System. Be an advanced practice clinician with expertise in CMS star ratings, quality indicators, program eligibility, Veterans Care agreements, facility waivers, and approvals. Work Location: 2285 Challenger Way, Santa Rosa, CA 95407. Schedule: Full-Time, Monday-Friday, hours to be determined. Remote work is not authorized. Relocation and recruitment incentives are authorized. EDRP is authorized, but former participants are ineligible.
