Job Details

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team in Freeport, NY (11520). In this role, you will:

Provide direct nursing care to residents following individualized care plans

Administer medications and treatments accurately and on time

Monitor and document residents’ health status and report changes promptly

Maintain compliance with all facility policies and healthcare regulations

Collaborate with interdisciplinary team members to deliver high-quality, person-centered care
