Reports To: Executive Director About Luminary Hospice:

We are centered on caring for the whole person – with expert medical care, pain management as well as emotional and spiritual support along with resources, information, and emotional support for families and caregivers. About the role:
Responsible for managing all aspects of the patient intake process including establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes. Job Responsibilities:
1. Coordinates all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies. 2. Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements. 3. Implements improved work methods and procedures to ensure patients are admitted in accordance with policy. 4. Establishes and maintains positive working relationships with current and potential referral sources. 5. Ensures seamless transition of patients to home care by providing patient education and preparation for home hospice care, plan of care initiation, and coordination of care with multiple service providers. 6. Ensures maximum third-party reimbursement through oversight of insurance verification and authorization processes. 7. Builds and monitors community and customer perceptions of Luminary Hospice as a high-quality provider of services. 8. Gathers, collates, and reports referral statistics including key customer referral trends. 9. Provides input to help identify opportunities for additional or improved services to meet unmet customer needs. 10. Maintains comprehensive working knowledge of Luminary Hospice of Indiana, LLC contractual relationships and ensures that patients are admitted according to contract provisions. 11. Participates in quality assessment performance improvement teams and activities as assigned. 12. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Luminary Hospice. 13. 1. At least three (3) to five (5) years' experience in health care preferred. 2. Ability to market and deal tactfully with internal/external customers and the community. 3. At least 1 year of experience with Medicare Hospice Certification as well as CHAP guidelines, preferred. 4. Proficient in MS Office applications and ability to learn department and job-specific software systems. 5. Demonstrate organizational skills. 6. Demonstrate effective verbal and written communication skills. 7. Demonstrate analytical skills when problem-solving. 8. Demonstrate high attention to detail and a high degree of accuracy.
