About the Role:

Key Responsibilities:
- Conduct initial patient evaluations and develop individualized treatment plans
- Provide hands-on therapy to improve mobility, strength, and overall function
- Educate patients and caregivers on exercises and techniques to support recovery
- Maintain accurate and timely documentation in accordance with home health regulations
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes
- Manage a caseload of 30 points per week
- Licensed Physical Therapist (PT) in the state of Florida
- Home Health experience preferred
- Proficiency with EMR systems (HomeCare HomeBase, Kinnser WellSky, or similar)
- Strong communication and organizational skills
- Ability to work independently and travel between patient homes
- Flexible schedules (Per Diem or Full-Time)
