About the Role:
We are seeking a skilled and compassionate Physical Therapist (PT) to join our Home Health team. The ideal candidate will provide high-quality rehabilitative care to patients in the comfort of their homes. This role offers flexibility, competitive compensation, and mileage reimbursement .
Key Responsibilities:

• Conduct initial patient evaluations and develop individualized treatment plans
• Provide hands-on therapy to improve mobility, strength, and overall function
• Educate patients and caregivers on exercises and techniques to support recovery
• Maintain accurate and timely documentation in accordance with home health regulations
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes
• Manage a caseload of 30 points per week

Qualifications:

• Licensed Physical Therapist (PT) in the state of Florida
• Home Health experience preferred
• Proficiency with EMR systems (HomeCare HomeBase, Kinnser WellSky, or similar)
• Strong communication and organizational skills
• Ability to work independently and travel between patient homes
