Your Role
- Provide Medicare annual health assessments in clinics and patient homes
- See 78 patients/day in clinic , 35/day on home visit days
- Perform H&Ps, use preventive screening tools, and document in EMR
- Collaborate with PCPs to ensure continuity of care (not a PCP role)

Requirements
- Active NP license (California)
- Bilingual in Vietnamese, Cantonese, or Mandarin
