Position Overview
We are seeking a compassionate and team-oriented Physical Therapist to join our renowned CHA PACE care team. In this unique role, you will provide skilled physical therapy services to older adults in their homes and in our outpatient gym located within the Adult Day Health Center. This position emphasizes proactive and preventive care, setting us apart from traditional, reactive home care models.
As a Physical Therapist in the CHA PACE program, you will:
•Collaborate closely with an interdisciplinary team to assess, plan, and implement individualized care plans.
•Provide direct therapeutic interventions in both home and clinic settings.
•Contribute to team meetings and ongoing care coordination discussions.
•Support participants in achieving optimal physical function and independence.
•Work without the burden of OASIS documentation, focusing instead on personalized care delivery and functional outcomes.
With a focus on quality care, service excellence, cultural responsiveness, and performance improvement, this role is ideal for a Physical Therapist who is passionate about geriatric care and preventive healthcare models.
Qualifications
•Bachelors, Masters, or Doctoral Degree in Physical Therapy required
•Massachusetts PT License
•Maintains current American Heart Association BLS for Healthcare Providers certification
•At least 1 year PT experience preferred
