Key Responsibilities
- Perform patient assessments and monitor vital signs
- Administer prescribed medications and treatments
- Assist with clinical procedures such as wound care and catheterization
- Collaborate with RNs, physicians, and therapists to implement care plans
- Accurately document patient progress and report changes
- Promote rehabilitation goals through compassionate care
- Ensure adherence to infection control protocols and facility standards
Requirements
Qualifications
- Education: Graduate of an accredited LPN program
- Licensure/Certifications:
- Active West Virginia LPN license
- Current BLS certification
- Experience:
- At least 1 year of experience in skilled nursing, rehabilitation, or long-term care
- Technical Skills:
- Proficient in medication administration, wound care, and EHR documentation
- Soft Skills:
- Strong communication, empathy, and adaptability in fast-paced environments
