What We Provide Attractive sign-on and referral bonus opportunities Generous paid time off starting at 30 days plus 9 company holidays Comprehensive health insurance for you and your family, including Medical, Dental, Vision, Life, and Disability Employer-matched retirement savings plans Personal and financial wellness programs Pre-tax flexible spending accounts for healthcare and dependent care Generous tuition reimbursement for qualifying degrees Opportunities for professional growth and career advancement Internal mobility options, CEU credits, and advancement opportunities A collaborative network of colleagues through the VNS Health Social Services Community of Professionals What You Will Do Conduct assessments to identify clients' needs and develop ongoing clinical care plans Manage a caseload effectively, leading care coordination for complex psychiatric cases Provide supportive counseling and ongoing mental health services Collaborate with and refer clients to appropriate agencies as necessary Develop a resource inventory to meet identified client needs Coordinate service providers and facilitate case record reviews Advocate and educate clients on available entitlements and community services Help clients establish support networks to prevent crises Participate in ongoing training to enhance clinical skills Maintain accurate case records in the program's EMR Join team meetings and discussions around care planning and discharge processes Participate in 24/7 on-call coverage as needed Serve as a liaison with community agencies Provide short-term counseling and risk assessments Collect and report necessary data adhering to productivity standards Lead network meetings using the Open Dialogue Model Qualifications Master's Degree in Social Work, Psychology, Mental Health Counseling, or related field At least two years of experience in mental health services for clients with serious mental illness or related issues Strong oral, written, and interpersonal communication skills Bilingual skills may be preferred based on operational needs Current license to practice in New York State as a Mental Health Counselor, Marriage and Family Therapist, Clinical Social Worker, or similar Valid NYS ID or driver's license, as required by operational needs Pay Range : USD $63,800.00 - USD $79,800.00/Yr.
