Responsibilities
In collaboration with the multidisciplinary team, addresses the social and emotional adjustments and complications of illness which result in barriers to treatment, risks to patient's health and safety and results in obstacles to discharge or continuing care. Interacts with patient, family and support systems to reduce barriers and promote progression of care. Provides assessment, counseling, information and referral, advocacy services and other resource assistance as needed.
Qualifications
Graduate of a Social Work Program, MSW required. 3 to 5 years experience. Current Iowa LMSW. Knowledge and skills necessary to provide services/care appropriate to the age and diagnosis of patients served across the continuum of care, in accordance with the hospital/departmental/unit standards. Maintains working knowledge of applicable Federal, State, and local laws and regulation as well as other policies and procedures in order to ensure adherence in a manner that reflects honest and professional behavior.
