GENERAL SUMMARY:

Plan, organize, monitor, and carry out Antimicrobial Management and Stewardship programs to optimize clinical outcomes, promote cost-effective therapy, and improve care while minimizing adverse effects of infectious disease treatment in the hospital. Integrate these programs with patient safety and quality improvement initiatives as outlined by regulatory agencies.

Responsibilities:

  • Maintain and disseminate drug information related to infectious diseases, including in-service education, consultations, and conferences.
  • Provide therapeutic guidance and drug information to hospital staff.
  • Manage treatment for diverse patient populations (adult, pediatric, geriatric, adolescent, psychiatric).
  • Monitor drug therapy for appropriateness and safety through pharmacy records.
  • Perform pharmacokinetics, IV to PO conversions, therapeutic interchanges, and other clinical pharmacy functions.
  • Develop and maintain reports for pharmacy management and antimicrobial stewardship, including antimicrobial utilization, hospital stay trends, and resistance patterns.
  • Serve as a consultant to hospital committees.
  • Participate in clinical rounding, public speaking, and data presentation.
  • Contribute to the development and evaluation of clinical pathways in infectious disease management.
  • Collaborate with pharmacy, infection control, information systems, and medical staff.
  • Ensure quality initiatives meet accreditation standards.
  • Report daily to the Clinical Pharmacy Manager.
  • Work rotating shifts (AM & PM).

Qualifications:

Knowledge: Pharm.D. or equivalent, licensed pharmacist, preferred residency or fellowship, advanced certification in antimicrobial stewardship.

Skills: Knowledge of pharmacy laws, medication technology, excellent communication, team participation, pharmacology, and problem-solving skills.

Abilities: Ability to work independently, handle stress, prioritize tasks, communicate effectively, and foster teamwork.
