We strive to hire excellent people at Concordia - and keep them as well!Full-time: Bellevue & surrounding areas (float).The Licensed Practical Nurse (LPN) works under the guidance of the Registered Nurse (RN) as a member of a team to give direct nursing care to patients requiring intermittent visits. The LPN collaborates patient care with the case managing RN, physician, and patient and follows a patient specific treatment plan.QUALIFICATIONSGraduate of state-approved school with course in pharmacologyCurrent license in the State of PennsylvaniaMinimum of two [2] years experience in patient careReliable, independent means of transportation to make home visits. Drivers must have a valid driver's license and drive a vehicle with insurance limits as established by the stateDUTIES AND RESPONSIBILITIESClinical SkillsProvides skilled nursing care that includes activities such as teaching and treatments (including IV therapy- if IV certified, venipuncture and complex wound care) and health promotion practice Executes preventive, rehabilitation and restorative nursing procedures appropriate for the patient's care and safetyAssesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs, changes in patient's physical, mental or emotional condition to the physician and the supervising Registered NurseEvaluates the patient and home situation to determine the health teaching required at each skilled visit.Instructs and educates the patient and the designated representative regarding the care requirements of the patient in the home.
