Job Summary: Under the direction of a licensed provider (e.g., RN, Physician) and as a member of a Virginia Mason Franciscan Health (VMFH) multi-disciplinary team, this job is responsible for serving as the admissions, discharges and transfers (ADT) nurse for patients across the acute care setting in accordance with established patient care standards and applicable regulatory requirements. An incumbent provides patient care, within scope of licensure, during the transition points of admission, discharge and transfer in support of efforts to optimize patient throughput. Key responsibilities include completion of nursing admission assessments, facilitating preparation and discharge of patients and supporting patient inter-department transfers. Job Functions: Under the direction and supervision of a Registered Nurse, performs nursing admission assessments on VMFH patients as they are admitted through the Emergency Department, surgical or procedural setting or through the direct admit process. (50%) Receives notification of pending admissions per established procedures; interviews patients to gather relevant information (e.g., weight, height, vital signs, drug allergies, presenting problems, etc) and make an initial assessment; screens, communicates and responds to patients' needs appropriately and within scope of practice. Enters patient data per established procedures in the electronic medical record. Ascertains pertinent information regarding a patient's current medication profile and communicates to receiving nurse in a timely and accurate manner. Observes general and specific physical and behavioral signs and symptoms; assesses for, and effectively communicates deviations from normal conditions; recognizes urgent/emergent situations and refers them to a RN or other provider as appropriate. Under the direction and supervision of a Registered Nurse, facilitates patient throughput by supporting the patient discharge process across the acute care setting. (35%) Performs patient discharge activities following established procedures; follows up with patient after discharge as necessary. Provides reinforcement of information and education through review of discharge instructions while discharging patients. Communicates what aspects of the discharge process have been completed to the responsible staff nurse. Performs nursing handoff to receiving facilities for patient's discharge into a post-acute setting. Under the direction and supervision of a Registered Nurse, supports inter-unit patient transfers across the acute care setting in order to facilitate throughput and bed availability. Transports patients to and from nursing units and ancillary departments in a manner conducive to patient safety and comfort; returns all equipment after transport (ie. wheelchair, stretcher); informs floors when patient is being transported; provides feedback to healthcare staff on occurrences during period of transport. Utilizes proper body mechanics to avoid workplace injury while transferring patients. Within scope of practice and under the direction and supervision of an RN, helps to provide nursing care and support to patients and care teams. Qualifications Education/Work Experience Requirements: Graduation from an accredited Practical Nursing program (or equivalent recognized military training) and successful passage of the National Council Licensure Examination for Practical Nurses (NCLEX-PN). Possession of a current Washington State license to practice as a Licensed Practical Nurse. Current healthcare provider BLS certification. NOTE: Commitment to completing an RN degree is a job requirement and incumbents must produce confirmed acceptance into an accredited RN program within nine months of date of hire to be eligible for continued employment in this position. Successful completion of the RN program is expected within two years of acceptance into the program.
