The ambulatory care clinical pharmacy specialist will serve alongside already established PharmDs as an integrated multidisciplinary team member within the Medical University of South Carolina's Infectious Diseases Clinic. The clinical pharmacist will work with patients and Infectious Disease team members to ensure effective clinical and pharmaceutical outcomes via medication therapeutic management and counseling for patients living with HIV while providing direct patient care to empower patients to actively participate in their clinical decision-making. The Clinical Pharmacist will provide pharmacotherapy management decisions and disease state counseling to people living with HIV for various conditions such as smoking cessation, diabetes, hypertension, and dyslipidemia, among others, via in-person or virtual/telephone visits. Assist in deriving ART regimens if there are resistance or adherence/toleration issues. See all patients newly diagnosed with HIV for medication and disease state education and provide first dose of medication in clinic if desired. Communicate and answer pharmacy-related questions from staff and providers. Provide primary care and health maintenance recommendations. Initiate injectable regimens for HIV (appropriateness of use, counseling to patients). Research and Education: Provide HIV education to the Infectious Disease Fellows, providers, and staff while in the clinic and through educational opportunities provided through the division. Provide didactic lectures through the College of Medicine, College of Pharmacy, and College of Health Professions. Work with the other HIV clinical pharmacy specialists in providing updates for the Antimicrobial Stewardship website publication consisting of an ARV card, single-rx card, renal dose card, and crush guide. Scholarship responsibilities expected. Administration: Assist medication nurse or provider with peer-to-peer reviews/appeals/medication acquisitions if insurance denies medication. Manage communication with patients, including phone calls and MyChart messages. Manage injectable HIV medications (maintaining doses within desired timeframes, ordering therapy plans, education to nursing staff). Attend community and division-based meetings to provide pharmacy input. Ryan White 340B: Conduct once monthly initial 340B audits.
