Solid Organ Transplant Coordinator

Corewell Health is committed to offering these advanced levels of care to our local community and beyond. Scope of Work: Coordinates transplantation of all recipients. Provides follow-up and care across the continuum from pre-transplant, during transplant and post transplant. Collaborates with members of the health care team to enhance patient care by developing, supporting and educating nursing staff. Makes clinical judgments based upon a synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems. An integral member of a larger interdisciplinary team contributing to excellence in patient care, research and teaching. Responsibilities include: Facilitating advanced nursing care for patients before and after transplantation along the continuum of outpatient to inpatient as required. Coordinating the evaluation process for pre-transplant patients and living donors (as applicable) to ensure effective use of hospital and/or clinical resources. Assessing post-transplant and post donation (as appropriate) organ function and general health status and collaboratively determining need for additional assessment or intervention. Participating in the development and implementation of protocol and standards of care for the transplant patient in collaboration with the multidisciplinary team. Providing on-call coverage including coordination of activities prior to transplant, the transplantation process when a donor is available (24/7), communication between physicians, local organ procurement organization, patients/families and the medical team involved, blood bank in collaboration with the physician on-call. Completing necessary documentation according to the United Network of Organ Sharing (UNOS) guidelines including listing, status changes and post-transplant/post-donation (as applicable) follow-up information. Communicating with multidisciplinary team members and patients and families in a timely manner in regard to patient care/issues and to develop discharge plans for patients pre-transplant or post-transplant including referrals for necessary discharge care. Maintains patient transplant record with current data to facilitate communication. Educating patients, caregivers and hospital staff along the continuum by developing patient education materials or identifying external resources that are appropriate to assist understanding of processes and procedures surrounding pre-, peri and post-transplant issues. Qualifications: Required Bachelor's Degree Nursing Preferred Master's Degree Nursing 5 years of relevant experience nursing experience required, with at least two years in transplant or critical care or clinical nursing experience in transplant or critical care preferred Required Program development Preferred LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required Or CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required CRT-Oncology Certified Nurse - ONCC Oncology Nursing Certification Corporation Blood and Marrow Transplant Upon Hire required CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association Solid Organ Transplant

Adult 1 Year required CRT-Clinical Transplant Coordinator (CCTC) - ABTC American Board for Transplant Certification Solid Organ Transplant Upon Hire preferred CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association Solid Organ Transplant -Pediatric Heart 1 Year required Ideal candidate will be an RN possessing 5+ years of RN experience and 2+ years of Critical Care Nursing experience Ideal candidate will possess 3+ years of Transplant or Donation experience This is an On-Site RN Position that is Monday to Friday, 8 a.m. to 4:30 p.m.
