Case Manager
The Case Manager supports the mission of Student Wellness and Counseling (SWC) by helping students gain access to and coordinate mental health and counseling care. The Case Manager's primary focus is assisting students in successfully accessing and navigating off-campus mental health services. Additional areas of focus include cross-service collaboration with Student Health, Student Disability Services, and other campus partners. The Case Manager develops and maintains systems to assist with referral care coordination and provides direct clinical services as needed. Responsibilities include: Assisting students in accessing mental health resources outside of SWC. Developing and implementing systems for follow-up with students who have been referred to community resources. Developing and maintaining a database of area providers and programs. Cultivating relationships with area providers and programs. Assisting students with resource access outside of the Chicagoland area. Conducting clinical diagnostic evaluation and assessment of clients, including urgent-care/crisis evaluations as-needed. Providing goal-focused, time-limited psychotherapy as-needed. Providing emergency and crisis intervention services during and after-hours as-needed. Maintaining timely and complete electronic documentation. Providing consultation, training, and outreach to members of the University community both during and after-hours as-needed. Participating as a member of the clinical team. Participating in clinical and administrative staff meetings as appropriate. Providing clinical supervision of trainees as needed and as appropriate. Ability to exercise discretion and confidentiality while handling sensitive situations. High attention to detail and ability to manage data. Ability to review, analyze, interpret, and/or prepare federal and state laws, rules, regulations, NASW Code of Ethics, policies, and procedures as related to social work required. Interprets, monitors, and record client progress according to measurable goals described in treatment and care plan. Plans own resources to combat social problems, prevent substance abuse, or improve community health and counseling services. Assesses the needs of individuals, performs adaptive tests and psychological screening, provides family counseling, and participates as a member of a developmental team. Identifies community and University resources and makes appropriate referrals. Documents developmental care in records. Delivers emergency and crisis intervention services. Performs other related work as needed. Minimum qualifications include a college or university degree in a related field and 5-7 years of work experience in a related job discipline. Preferred qualifications include an MSW from an accredited institution, experience in a university counseling setting, and clinical care and case management experience. Licensed or license-eligible in the State of Illinois for clinical social workers is also preferred. Application documents required include a cover letter, resume, and list of three references.
