What You Bring To SBHG (Qualifications): 4 years of experience in a social service field OR Associate Degree plus 2 years of experience in a social service field OR Bachelor's degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service Valid California Driver's License Note: Must possess a valid California Driver's License and maintain a driving record that meets the company driver's eligibility policy. How You Will Make A Difference: This position has primary responsibility for case management services to clients and their families referred through Stars' Transition Age Youth (TAY) Program (TAY). Division/Program Overview: Outpatient, community-based program that serves youth age 18-25 with serious and persistent mental health issues Program provides intensive case management, outreach and engagement, medication support, as well as, mental health counseling, peer groups, and supportive services to assist TAY transition to adult life.
