CHHUNY High Needs Care Manager
Finger Lakes Region, NY (Onsite) Full-Time $20.00 - $22.00/Hour Job Summary: The CHHUNY High Needs Care Manager serves as the primary coordinator for children and youth with complex needs within the Children's Health Home of Upstate New York (CHHUNY) network. This role is essential in helping families navigate medical, behavioral, and social service systems through comprehensive care planning, consistent follow-up, and a collaborative team approach. You'll act as the central communication hub, ensuring all providers are working toward the best interest of the child or youth. Minimum Qualifications To serve high-acuity enrollees, applicants must meet one of the following: Bachelor's degree in a related field with two years of relevant experience Licensed Registered Nurse (RN) with two years of relevant experience Master's degree in a related field with one year of relevant experience Skills & Competencies: Strong interpersonal and communication skills, including the ability to engage with diverse populations Solid knowledge of medical, behavioral health, and community support systems Excellent time management, problem-solving, and multitasking abilities Ability to work independently and within an interdisciplinary team Experience with electronic health records and Microsoft Office (Word, Excel, Outlook) Must be comfortable providing services in community-based settings Typing proficiency of 30+ WPM What We're Looking For: Passionate about improving outcomes for children and families A strength-based, trauma-informed, and culturally responsive approach to care A team player with flexibility, initiative, and a collaborative mindset Job Requirements: Conduct comprehensive assessments to evaluate the child/youth's strengths, needs, and risk factors Develop and maintain person-centered care plans in collaboration with the youth and their parent/guardian Coordinate services across providers, including medical, behavioral health, social services, education, and housing Maintain frequent communication with the care team to ensure collaborative, coordinated care Utilize telehealth tools and mobile technology to increase engagement and continuity of care Advocate for client needs and facilitate access to needed services and supports Ensure timely documentation and compliance with Health Home and agency standards Provide transportation or use personal vehicle to meet clients in the community, as needed Deliver care management services across a seven-county region, including occasional evening or weekend on-call shifts About Us: Finger Lakes Area Counseling and Recovery Agency (FLACRA) is a growing, not-for-profit provider of behavioral health and substance use treatment services throughout the Finger Lakes Region.
