About the Role

Under the direction of an RN, participates in the provision of direct resident care for assigned residents within the framework of licensure requirements. Functioning primarily at the bedside, performs a wide variety of resident centered activities utilizing established policies and procedures mainly with adults throughout the stages of adulthood. In addition, guides and supervises Certified Nursing Assistants in assigned areas. This position will care for patients in the adult and geriatric age groups. Employee performs clinical duties in accordance with population specific guidelines and adheres to National Patient Safety Goals as outlined in policy and procedures. Minimum Job Qualifications

Licensure or other certifications: Currently licensed as an LPN in the State of Georgia. Current BLS certification required or must be obtained within 30 days of hire.

Educational Requirements: High School Diploma or GED. Graduate from a state approved LPN school of nursing.

Preferred Experience: Two (2) years experience in Long Term Care or Medical Social Services.

Knowledge of basic nursing skills acquired at LPN/RN program

Interpersonal and leadership skills

Essential Tasks and Responsibilities

Has knowledge of the nursing care activities required to provide appropriate patient care and to assess the resident. Explains procedures or care to be provided to resident prior to starting procedures. Completion of Braden scale. Performs weekly skin assessment. Competes report to the Director of Nursing. Performs weekly documentation per policy and procedure. Assesses resident's skin condition on admission; hospital returns and when change in resident's care. Reports changes to Director of Nursing immediately. Provides information thru skin assessments and reports that is used to update the nursing care plans and Resident Information Sheets. Contributes to the updating of the nursing care plan. Demonstrates effective time management by completing care assignments, documentation, and other duties within scheduled shifts.

Restorative Nursing. Monitors the application and correct positioning of splints and other positioning devices. Monitors restorative programs and documentation.

Other duties as assigned.
