The Licensed Practical Nurse (LPN) participate in the implementation and evaluation of patient/resident care for stable and predictable patients/residents of all ages in the home care setting. Position Summary

The Licensed Practical Nurse (LPN) in Home Health provides direct nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN). The LPN is responsible for delivering compassionate, safe, and effective care that is aligned with the patient's individualized plan of care, while recognizing the unique needs of patients across the lifespan. The LPN demonstrates clinical competency, sound judgment, and a commitment to promoting health, comfort, and safety in the home setting. Key Responsibilities

Deliver skilled nursing care and treatments as ordered by the physician and delegated by the RN, in compliance with home health regulations and standards. Collect and assess data reflective of the patient's status and contribute to the development and revision of care plans. Administer medications and treatments safely and accurately, while monitoring for side effects and effectiveness. Monitor patient progress, identify changes in condition, and promptly report to the supervising RN or physician. Document all care and interventions thoroughly, accurately, and timely in the electronic medical record. Educate patients and family members on disease processes, medications, treatments, and home care techniques to support self-care and prevent complications. Provide age-appropriate and culturally competent care, demonstrating awareness of population-specific standards. Maintain effective and professional communication with patients, families, physicians, and interdisciplinary team members. Ensure patient safety in the home environment by recognizing hazards and implementing preventive measures. Participate in team meetings, case conferences, and performance improvement activities as required. Maintain patient confidentiality and adhere to ethical and legal standards in all aspects of practice. Qualifications

Graduate from an accredited practical nursing program as a Licensed Practical Nurse. If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing. One year of professional nursing experience preferred. Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges. Obtains and subsequently maintains required department specific competencies and certifications. Depending on location, a valid drivers license may be required. Benefits
