Health Facility Evaluator II

The primary function of the Health Facility Evaluator II is to conduct inspections utilizing an established survey process. The survey process includes evaluation of health care facilities for state licensure and federal certification requirements for participation under Medicare/Medicaid. The incumbent will document findings and complete written reports of survey findings. The incumbent may be required to perform surveys as an individual, team member or team leader. The incumbent will conduct on-site health facility inspections primarily in these 18 panhandle counties located in Northwest Florida: Madison, Taylor, Jefferson, Leon, Wakulla, Gadsden, Liberty, Franklin, Gulf, Calhoun, Jackson, Bay, Washington, Holmes, Walton, Okaloosa, Santa Rosa, and Escambia. As assigned by management, this position is required to successfully complete the Surveyor Minimum Qualifications Test within the first 12 months of employment as required by Sections 1819(g)(2)(c)(ii) of the Social Security Act, as amended, and Article IV(B) of the Agreement pursuant to Section 1864 of the Social Security Act. This position has been identified as mission essential. The incumbent in this position may be required to work during the weekend or on holidays. In addition, mission essential personnel will be required to work during disasters, to include but not limited to, work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency work may involve the incumbent to work in another county or staffing location to assist other State Agencies with emergency work. Emergency duties may include, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural. The incumbent must be proficient with computer equipment and software programs, such as Microsoft Word and Outlook, as well as smart phone technology. The incumbent must also be available to work before and after normal work hours including weekends and holidays. This position may require extensive travel, including significant overnight travel. Successful completion of a criminal background investigation is a condition of employment. A good attendance record is essential for any individual in this position as the work involved occurs daily and is time-critical. The individual in this position is expected to report to work daily and on time. Leave must be requested in advance except in emergency circumstances not under the control of the employee.
