We are seeking a full-time RN Coordinator to join our team! The RN Coordinator is responsible for building and maintaining the withdrawal management (245F) program for clients. The RN Coordinator will assist in integrating the substance use programs by establishing appropriate referrals, enlisting client participation in the program(s), and coordinating care with the assistance of a multidisciplinary team.
Responsibilities
Facilitates and monitors internal and external treatment referrals. Coordinates and facilitates communication between clients, primary care providers (Physician, Nurse Practitioner or Physician's Assistant), behavioral health providers, and any other providers. Provides education on medication management and compliance. Educates and trains staff per MN State Requirements, license regulations and per ZVHC Policies and Procedures. Triages potential Commitment Admissions. 24/7 On-call for nursing questions, concerns and triage as needed. Assist with the development and continuity of implemented projects identified and/or delegated. Oversee staff education requirements and needs and report any issues to the Program Supervisors. Overseeing and serving as a staff resource and coordinator of the 245F Withdrawal Management Program.
Requirements and Qualifications
Must be a graduate of an accredited school of practical nursing. Active Registered Nurse by the Minnesota Board of Nursing and abides by the Nursing Practice Standards. One year of experience working in a mental health or chemical dependency setting required. Experience working with an interdisciplinary team approach and using an electronic medical record.
