Position Highlights Effectively and efficiently plans visit content and schedule based on client capabilities, condition, preferences (when possible), physician orders and available resources.

Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical, and professional behavior.

Performs specific treatments and administers medications to implement the medical and nursing care plans.

Apply appropriate preventive, therapeutic, and rehabilitative nursing procedures, and techniques to clients.

Utilizes interview, observation, and evaluation in assessing clients and applies nursing judgment, consistent with practice standards, in making recommendations to the primary nurse and client.

Reports changes in client condition as appropriate and in a timely manner, to the clients physician and/or Case Manager/designee and obtains orders for changes in the plan of treatment to respond to the clients condition.

Instructs family members in client care.

Assists in care plan revisions as indicated.

Participates in care conferences with other members of the health care team.

Utilizes and implements appropriate tools and pathways per guidelines and/or policy and procedure (e.g. education checklists, pathway protocols, Home Program Instructions, etc.).

Minimum Qualifications Graduate of an accredited or state approved school of practical nursing

Licensure as a LPN in the State of Michigan

A minimum of five (5) years of experience in an acute care setting; home care experience preferred.

Phlebotomy and wound care experience required.

Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
