Job Description Job Title: Registered Nurse (RN)
Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: 8 am to 5 pm; potential for on call from 5 pm to 8 am 1-2 times per month Hours per Shift: 8 hours Experience: Minimum of one (1) year as an RN, either in acute care hospital, oncology, long-term care, hospice, or home health. School internships and clinical hours may be considered as experience in individual cases. License: Current license to practice nursing in the state of employment. Certifications: BLS (American Heart Association); current automobile insurance and valid driver's license. Must-Have: Knowledge of terminally ill patients and their families; understanding of hospice; ability to apply knowledge of the special needs of hospice patients and families.
Description: The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient's family. The RN will manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance the quality of hospice care. The RN will case manage all assigned patient clinical records. The RN will ensure the highest quality of care is provided and documented in accordance with the Plan of Care. The RN will perform the initial and ongoing assessment of the patient. The RN will help develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with the other members of the interdisciplinary group. The RN will work closely with other members of the interdisciplinary group to provide comprehensive physical, psychosocial, educational, emotional, and spiritual services to achieve the highest quality of care. The RN will report any change in the patient's condition to both the attending physician and hospice physician. The RN will maintain an accurate and updated medical record, including all interdisciplinary notes, Plans of Care, initial and ongoing assessments, and any other information relevant to the care of the patient. The RN will practice infection control measures and standard precautions. The RN will monitor scheduling of all staff. The RN may be requested to participate in the agency on-call schedule. The RN will consistently promote the company's core values. This is a safety-sensitive position. The RN will complete required compliance annual training. The RN will perform other activities as assigned. The RN will attend staff meetings. The RN will maintain a high level of knowledge pertaining to the management of pain and symptoms in the dying process. The RN will make routine supervisory visits with all other team members at least monthly. The RN will perform ongoing review and follow-up on assigned patient clinical records to ensure quality services are provided and standards of practice are followed. The RN will maintain close contact with the patient's family and/or significant others to provide information, support, and continuity of care. The RN will maintain primary control and professional management of each patient and act as the primary liaison between physician, patient/family, and the hospice team. The RN will be fully aware and abreast of all clinical issues, staffing, and scheduling concerns. The RN will be responsible for the education of the patient, family, or other caregivers concerning needs identified in the Plan of Care, including medication administration, dressing changes, oxygen administration, hospice philosophy, and care of the terminally ill patient.
